Required Enrollment Documents

To enroll in Claremore Public Schools, we must have the following documents presented:

Two proofs of residency showing proof of physical residency including 1 current utility bill or deposit receipt and a lease agreement, mortgage statement or property tax document.
Original birth certificate
Withdrawal from prior school. Students in grade10-12 need a transcript. Ninth grade need a report card from previous school
Updated immunization records from the health department or doctor’s office. State of Oklahoma require Hepatitis A and Hepatitis B shots
Proof of custody or guardianship if applicable
Parent’s driver’s license
Indian tribal card and/or CDIB card if applicable
