All CPS student-athletes can receive a sports physical at school for just $20 — quick, easy, and convenient!
📝 Important Reminder:
A parent/guardian must complete AND sign the medical history form before your student can be seen by the physician at school.
Don’t miss this opportunity to get cleared and ready for the season! WRJH students can walk over to the Doug Mosier Gym after school to have the physical.
