🎯Student-athletes in 7th-12th grade during the 26-27 school year are eligible for this service.
Required:
➡️Both sides of the Medical History and Parental Consent Form must be completed before the physical.
➡️Rank One consent forms must also be filled out before competing in athletics. Claremore Athletics Rank One Procedure. Complete Rank One Sports Online.
➡️CPS / OSSAA Sports Physical and Permission Form (Downloadable PDF)
